Job Description:
Sapphire Nursing at Meadow Hill, a long term skilled nursing facility located in Newburgh, New York is actively looking for a caring, dedicated Per Diem Physical Therapist to provide quality care to its residents. The Physical Therapist evaluates and treats patients/residents, communicates with their families, physicians, and other health team members, and maintains all necessary documentation of services in the patient s medical records. Physical therapy services include: treatment planning & implementation, discharge planning, testing patients physical abilities, analyzing rehabilitation goals, and consulting with the rehab team and other health team members to coordinate the most effective treatment program. PT Duties Include:Supervise the activities of physical therapy assistants.Evaluate Residents conditions & develop appropriate physical rehabilitation plans.Help Residents improve their mobility.Relieve Resident s pain using exercises, hands-on therapy & stretching.Teach Residents how to use physical therapy equipment.Help Residents through their tasks and monitor their performance.Maintain Resident confidence & protects the facility by keeping information confidential.Complies with federal, state, and local legal and professional requirementsContributes to team effort.Submit all billing logs and necessary information on a timely basis.PT Requirements Include:Graduate of an accredited Physical Therapy ProgramCurrent New York State PT LicenseNursing Home Experience preferredWe offer:Weekly Pay Checks!About Us:Sapphire Nursing at Meadow Hill considers the health and safety of its residents, family members, and team members as its highest priorities.
