Order#: NY1508530Physical TherapistHours of Employment: Full-time, 40.0 hours per week. Days and hours of work are Monday through Friday variable each day between the hours of 8:00am-5:30pm. Evening work may be required as job duties demand.Requirements: Bachelor's degree in Applied Science. Completion of accredited Physical Therapy program. License or eligible for license as a Physical Therapist in New York State. Current New York State registration as a Physical Therapist. Valid New York State driver's license. Experience working with individuals with developmental disabilities preferred. Proficient computer skills with knowledge of Microsoft Office programs. Must have demonstrated commitment to performing job duties and tasks at a high level of performanceSummary/Objective CP Rochester/Happiness House exists to support people with physical and developmental disabilities in choosing and accomplishing successive individualized life goals. We do this by providing a wide range of services to people of all ages in the Finger Lakes and surrounding greater Rochester area. Our purpose is to ensure that those we serve safely determine their own pathways in life and live as full members of their communities.Essential Functions: Imparts agency philosophy of Equal Opportunity, Independence and Realization of Individual Potential to children, families, program staff and community resources. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy. Upholds NYS Education Law/Practice Act as relating to the practice of physical therapy. Performs appropriate evaluations to individuals to determine the need for physical therapy services. Ensures valid prescriptions are written for all individuals on caseload. Carries out treatment plans developed after evaluation, in clinic setting for the Article 16 and Article 28 outpatient clinics as well as the Preschool or School Programs, as necessary. Available to consult with other professionals such as teachers, habilitation staff, as well as other members of the interdisciplinary team as appropriate, to facilitate delivery of coordinated quality services to individuals. Works with legal guardians, house staff or parents as integral members of the interdisciplinary team in the clinic, day hab or school setting, as needed. Communicates with physicians, orthopedists and other medical personnel in order to facilitate evaluation and treatment of individuals. Completes all paperwork in accordance with agency, program and department policies/guidelines. Ensures knowledge of paperwork, Electronic Health Record and applicable Medicare and Medicaid regulations in order to provide appropriate documentation. Attends all team, discipline and staff meetings as designated. Maintains treatment area in an orderly manner to ensure safety. Maintains all department equipment as appropriate. Advises Supervisor of Clinical staff of need for replacement of old or ordering of new equipment. Meets with interdisciplinary team and Program Supervisor as needed to review status and need of program modifications for individuals on caseload Participates in conferences, workshops and in-service training as needed or recommended to enhance job skills and personal growth and development. Adheres to agency's policies and procedures as well as department guidelines.
