We know that we can only achieve our vision by working collaboratively with our community to provide the care the community needs in a coordinated, seamless way.WHO YOU ARE:Under the general supervision of the Director of Rehabilitation and Diagnostic Services, our Physical Therapist is responsible for administering Physical Therapy services including evaluations, establishment/modification of treatment plan, provision/supervision of treatment and documentation to inpatient and outpatients within the departmental and facility policies and procedures. Thereby, helping to restores patient's function, alleviate pain, and prevent disabilities by planning and administering medically prescribed physical therapy.This is a full-time position working 30 hours per week. This position is eligible for participation in medical benefits.ResponsibilitiesClinical EvaluationValidates prescription from provider is present, understood prior to the evaluation and treatment of the patient. Provide evaluation and treatment according to the provider's order, calls provider if clarification is neededPerforms evaluations, and reevaluations according to the departmental policiesEvaluates effects of physical therapy treatments and fit of prosthetic and orthotic devices by observing, noting, and evaluating patient's progress; recommending adjustments and modificationsTherapist will use objective measures on all evaluations and documented findings. Measureable/functional goals will be written based off these objective findingsTreatment plan will be developed with the input of the patient and according the providers orderTreatment and Discharge PlanTreatment plan addresses the needs of the patient including age, physical, psychological and learning needs. Encourages the patient to participate in their plan of care and goals by asking appropriate questionsUses a variety of therapeutic exercise, functional training, manual therapy techniques, assistive and adaptive devices and equipment, and physical agents and electrotherapeutic modalities to treat patientsEducates patient of ongoing goals, as they are being achieved or modified and of impending dischargeCompletes discharge planning by consulting with physicians, nurses, social workers, and other health care workers; contributing to patient care conferencesAssures continuation of therapeutic plan following discharge by designing home exercise programs; instructing patients, families, and caregivers in home exercise programs; recommending and/or providing assistive equipment; recommending outpatient or homeQuality and Customer serviceGreets and welcomes all patients by name with a smile and on timeEnsures documentation is complete and thorough with all patients in accordance with departmental policies and expectations of patient's insurance requestsCompletes and edits own charges on a daily basis to help ensure accuracyParticipates in mandatory competencies yearly as demonstrated by attendance and test and participates in monthly chart auditsMaintains professional and technical knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; participating in professional societiesDevelops physical therapy staff by providing information; developing and conducting in-service training programs.OtherComplies with federal, state, and local legal and professional requirements by studying existing and new legislation; anticipating future legislation; enforcing adherence to requirements; advising management on needed actionsKeeping current with Insurance demands, restrictions, authorizations that may be needed, educating and informing patients of their insuranceTeach/ lead physical therapy students as well as those in other health professions. Educate other healthcare providers of Physical Therapy and what they can provideDirect and supervise PTA, delegating specific patients to them and establishing channels of ongoing communicationConsult and practice with other health professionals for the overall wellbeing of the mutual patientWill travel to off-site approved locations as needed to provide therapy services and other Wellness programsMINIMUM EDUCATION AND WORK EXPERIENCEEducation: Bachelors and /or Master's and /or Doctorate degree in Physical Therapy from an APTA -accredited institution.Experience: 3-5 years of experience in a hospital and outpatient setting.Licensed in NH by the NH State Board of Allied HealthCPR certifiedADDITIONAL INFORMATION: Hours: 30 or 40 hours per week.Location: Outpatient primary location (5 minutes from main hospital)Compensation: $38 - $52 per hour based on experienceBenefits: Medical, Dental, Vision, Retirement Plan and more!Location: Wolfeboro, NH 03894
