Outreach Rehab is a growing practice of in-home outpatient physical, occupational, and speech services in New York and New Jersey. Due to our continued growth, Outreach is looking to bring on additional motivated and passionate Therapists to work with adult and geriatric populations. We aim to bridge the gap between a home health agency and an outpatient clinic, providing all treatments in patients' homes, Senior Residences, Assisted Living Facilities, or office settings.
Responsibilities of Physical Therapist:
- Assess patients' home environments for safety and environmental barriers
- Provide fall risk assessments
- Address physical underlying impairments and how they affect overall function
- Provide ambulatory aid and adaptive equipment training
- Educate patients and caregivers regarding compensatory strategies and/or a home exercise program
Qualifications of Physical Therapist:
- Master's degree in Physical Therapy (minimum requirement)
- 1 year of experience working with older adults preferred
- Home care experience is a plus
- NY Physical Therapy License (required)
