Job Description
Essential Job Functions and Responsibilities:
* Performing and documenting initial evaluations and follow-up treatment visits in accordance with state practice act and consistent with current best-practices.
* Establishing appropriate plan of care for patients including prognosis, short/long term goals, and complete documentation in a timely manner.
* Prioritize a patient centered approach with a focus on patient's desired goals and function.
* Regularly reassess patient progress and modify plan of care as necessary.
* Communicate effectively with clinic staff, physicians, and other members of the patient's healthcare team
* Providing supervision for Physical Therapist Assistants, ancillary staff, students and volunteers as required
* Maintaining a full patient caseload and remaining flexible to accommodate patients. Follow-up treatment sessions are scheduled for 30 minutes one-on-one with each patient.
Requirements include:
* Bachelor's Degree; Master's or Doctorate Degree preferred
* 2-4 years of experience required; 5-8 years' preferred
* New graduates may apply if strong experience in outpatient setting
* Current NJ Physical Therapy License
* BLS Certification
* Ability to work independently or as part of a team required.
* Advanced certification (OCS, CSCS, COMT, MDT, etc or similar) preferred but not required
