Job Summary: Provides skilled physical therapy services in the center consistent with professional standards and policies. Participates in the development of programs necessary to achieve patient care objectives. This position is delegated the responsibility and accountability necessary to carry out assigned duties and adherence to Company's mission, values, policies, and procedures.
Job Duties:
Completes all documentation and billing required and in accordance with state and federal regulations and policy.
Performs patient screens, annuals, and nursing requests to determine the need for services and/or recommendations to care plan.
Provides updates on patient status, progress, discharge planning, as well as any follow-up needs or items to Director and Rehab team as needed.
Attends facility meetings as requested.
Develop a therapy plan of care.
Use standardized assessments to validate the need for skilled PT services.
Communicates and coordinates patient care with other members of the patient's healthcare team.
Displays competency in execution and application of various treatment modalities including but not limited to diathermy, e-stim, and ultrasound.
Provide patient and family with teaching related to patient's physical therapy program and it's maintenance.
Requirements:
Education is required in an accredited Physical Therapy program and a minimum of a Bachelors Degree. Qualification as a licensed Physical Therapist in the state(s) of practice.

Basic Computer Knowledge.
