Non-acute care is offered through various affiliates, including the Thompson House for rehabilitation and skilled nursing services, and the Home Care organizations.Summary of Job Purpose:Responsible for the evaluation, planning, directing and administering of physical therapy modalities of treatment as prescribed by a licensed physician. Administers treatments in an effort to restore function and prevent disability following injury, disease or physical disability. Assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities. Population served based on scope of services in the department.Essential Job Responsibilities:Thoroughly evaluates and sets plan of care to meet patient's identified treatment needs.Establishes objective, measurable and functional patient goals.Implements and updates treatment program to maximal effectiveness.Effectively oversees patient care delivery by assistants/aides.Completes all documentation within established time frames and according to regulatory guidelinesInvolves patient/family in establishing treatment plan and accommodates for individual needs (cultural differences, language, etc.)Participates in educational in-services, meetings and performance improvement activities as requested by Coordinator.Assists in the training/supervision of affiliating students/volunteers as requestedMaintains all department equipment and treatment areas in a clean and safe manner.Accurately enters patient charges/provides patient billing information to office staff.Displays analytical and basic problem solving skills.
