Salary Range $64,750-$85,000(Pay information will vary depending upon relevant experience for the position)Full Time 40 Hours GENERAL SCOPE: Plans for and provides Physical Therapy (P.T.) services in accordance with current applicable federal, state and local standards, guidelines and regulations, as may be directed by the Director to assure that the highest degree of quality resident care can be maintained at all times in accordance with the mission of the Jewish Home.EDUCATION / QUALIFICATION: Graduation from an approved school of Physical Therapy.Current licensure to practice Physical Therapy and currently registered in New York State or eligible for license in New York State.ESSENTIAL FUNCTIONS:
- Examines and interprets medical records.
- Performs detailed evaluations and screening.
- Establishes measurable goals.
- Develops and administrates an individualized treatment plan.
- Serves as a consultant to Nursing to assist them in the completion of the MDS+ form.
- SupervisesP.T. Assistant, Physical Therapy students and per diem Physical Therapists.
- All other duties as assigned.
