Position Summary: The Physical Therapist functions as an interdisciplinary team member providing patient assessment/re-assessment, direct patient care and individualized care planning/implementation.
Essential Functions include but are not limited to:
- Knowledge of the principles, methods, practices, techniques, equipment and procedures used in physical therapy
- Knowledge of anatomy and physiology sufficient to evaluate and treat patients
- Knowledge of equipment and the ability to adjust equipment to suite patient needs
- Collects and integrates data from multiple sources
- Applies critical thinking skills and considers alternatives
- Organizes and prioritizes multi-task workload
- Assesses health literacy and incorporates teaching with a focus on health promotion
- Plans, evaluates and administers physical therapy programs to out-patient populations
- Completes accurate, timely documentation, appropriately using computer charting/ordering
- Demonstrates department specific skills and competencies
- Provides supervision to licensed PTA's according to the state of NH practice act, reviewing notes and collaborating in care
- Complies with insurance and CMS regulations
- Performs all other duties as assigned.
- Attendance: works as scheduled and is compliant with Attendance and Tardiness Policy
Qualifications
Education & Qualifications:
- Licensed by the New Hampshire Board of Medicine as a Physical Therapist
- Bachelor, Master or Doctorate in Physical Therapy as required by the Federation of State Boards of Physical Therapy
- BCLS required
