Job description

Essential Job Functions/Responsibilities:

• Conduct assessments, treatment planning, evaluation of the response to care and readjustment of the treatment plan as needed;
• Plan and provide community education to patients, families and peers;
• Accurately document patient care treatments in EHR (i.e., HomecareHomeBase);
• Determine a PT treatment plan

Position Qualifications:

• 2 years of physical therapy work experience;
• Tech-enabled/Complete documentation timely
• Current & valid CA PT license; and
• American Heart Association Basic Life Support (BLS) certification.

Job Type: Full-time

Pay: $105,000.00 - $110,000.00 per year
