Location: Everett,WA, USA
This part time position will be traveling to see patients onsite and in the community of our Everett and Redmond sites.
The Physical Therapist (PT) is responsible for PT evaluations and development and implementation of the PT plan of care in accordance with PACE policies. The PT is a member of the Interdisciplinary Team and responsible to communicate and coordinate care with the team as well as multiple outside providers, caregivers and family members. The PT functions independently, working in multiple settings including the PACE center, participant homes, Adult Family Homes, Assistive Living Facilities and nursing homes. The PT assumes responsibility for participants treated by the PTA and PT aide.

Required Qualifications:
* Bachelor's Degree Physical Therapy.
* within 30 days of hire: National Provider BLS - American Heart Association.
* upon hire: Washington Physical Therapist License or upon hire: California Physical Therapist License - if practicing within this state
Preferred Qualifications:
* Master's Degree Physical Therapy or D.P.T. Physical Therapy.
* 1 year Working with a geriatric population.
