This session is for Experienced and New Graduate PTs & PTAs looking to learn more about Home Health. This is a unique opportunity to hear directly from our leadership and key Physical Therapy speakers at Catholic Health Home Care and ask any questions you might have! Job Details Our Home Care Physical Therapists: Review patient charts and medical history Develop a personalized treatment plan Teach therapeutic exercises and stretches to improve patient's conditions Use hands-on therapy to promote healing Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly Work closely with a supportive patient care team including physicians, nursing, social services, pastoral care, home health aides and others for your assigned caseload. Provide patient and family education for health promotion as directed by physician's plan of treatment. Provide care to the most upmost standard Minimum Qualifications: New York State PT license. New grads welcome! Valid New York State Driver's License and reliable transportation Our Home Health Physical Therapist Assistants: Follow established physical therapy plan of care, including physical therapy interventions and patient/caregiver education Document all care provided and maintain records per Catholic Health policy and standards. Assist in monitoring patient progress through observation and follows up with Supervising Therapist. Compile with scope of practice for PTA, including a co-visit with Physical Therapist on all initial PTA visits and keeps PT informed. Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions. Detailed documentation of visits in EMR system. Minimum Qualifications: Current NYS PTA License Basic Life Support Valid New York State Driver's License and reliable transportation Electronic Medical Record experience.
