Plans, implements, renders and evaluates physical therapy services in the home care setting using professionally approved assessment and therapeutic skills and modalities in accordance with the scope of practice for a Licensed Physical Therapist in New York State.

Bachelor Degree in Physical Therapy. One (1) year experience in Physical Therapy. Licensure to practice as a Physical Therapist in NYS. Valid NYS drivers license.

8:30a-5p variable/one weekend a month

**Salary Range:**

$55 per revisit; $165 per start of care; $25 an hour for edu/office time
