Join our dynamic team as a Physical Therapist in our Skilled Nursing Facility, where you'll make a significant impact on residents' lives. We're seeking a compassionate and skilled PT to develop and administer therapy plans, promote mobility, and enhance overall quality of life for our residents. New graduates are welcome, and experience in long-term care is a plus!
Key Responsibilities - Conduct thorough evaluations of residents' conditions upon admission
- Develop and implement personalized physical rehabilitation plans
- Utilize exercise therapy, manual therapy, and other techniques to improve mobility and reduce pain
- Educate residents on proper use of physical therapy equipment
- Monitor and document resident progress, adjusting treatment plans as necessary
- Collaborate with interdisciplinary teams to ensure comprehensive resident care
- Supervise physical therapy assistants when required
- Maintain a safe, clean, and confidential working environment
- Adhere to infection-control policies and protocols
- Comply with federal, state, and local regulations and professional requirements
Job Requirements - Graduate of an accredited Physical Therapy Program
- Current New York State PT License
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Commitment to ongoing professional development
- Experience in skilled nursing or long-term care (preferred but not required)
