Position Summary We are seeking a dedicated Physical Therapist to join our team. Our therapists travel to patients' homes, offering: Comprehensive evaluations and personalized treatment plans, Postoperative rehabilitation, Pain management, Mobility improvement and others. As a Physical Therapist, you will play a crucial role in helping patients recover from injuries, illnesses, or surgeries by developing personalized treatment plans and providing hands-on therapy. - Conduct assessments to evaluate patients' physical conditions and needs.
- Develop individualized treatment plans tailored to each patient's unique goals.
- Implement therapeutic exercises and techniques to improve mobility, relieve pain, and prevent or limit permanent physical disabilities.
- Educate patients and their families on at-home exercises and self-care techniques.
- Collaborate with other healthcare professionals to ensure comprehensive patient care.
- Maintain accurate and up-to-date patient records.
- This role requires travel to various locations, candidates must have their own method of transportation.
Knowledge, Skills and Abilities
- Strong physiology knowledge to design effective treatment plans.
- Familiarity with trigger point therapy for pain management.
- Ability to work effectively in both clinic and hospital environments.
Education & Experience
- Proficient in-patient care with a focus on geriatrics and hospital medicine.
- Experience in inpatient and outpatient settings, particularly in orthopedics.
- This position requires a valid Physical Therapy license for CA and CPR certification.
- Must have a reliable method of transportation and be willing to travel as needed.
