Summary:
Uses the therapeutic application of physical and chemical agents, exercise, and other procedures for the treatment and prevention of illness and injury; evaluates patient's abilities and needs; establishes plan of care and goals. Objective: achieve the highest possible level of functional independence for each patient - restore function and decrease potential for further disability. Population served based on scope of services in the department.
Responsibilities:
* Thoroughly evaluates and sets plan of care to meet patients identified treatment needs.
* Establishes objective, measurable and function patient goals.
* Implements and updates treatment program to maximal effectiveness.
* Effectively oversees patient care delivery by assistants/aides.
* Completes all documentation within established time frames and according to regulatory guidelines.
* Involves patient/family in establishing treatment plan and accommodates for individual needs (cultural differences, language, etc.)
* Participates in educational in-services, meetings and performance improvement activities as requested by Coordinator.
* Assists in the training/supervision of affiliating students/volunteer as requested.
* Maintains all department equipment and treatment areas in a clean and safe manner.
* Accurately enters patient charges/provides patient billing information to office staff.
* Maintain and Model REACH Values (Respect, Excellence, Accountability, Compassion, Honor)
* Demonstrates regular, reliable and predictable attendance
* Performs other duties as required
Other information:
Education: Graduate of an accredited curriculum with a minimum of a Doctorate of Physical Therapy. For individuals who completed appropriate Physical Therapy education prior to 2015, with a valid Physical Therapy license in the state of practice or proof of license eligibility, a Bachelor's degree or greater in Physical Therapy will be accepted in lieu of the Doctorate of Physical Therapy degree.
Current license to practice Physical Therapy : Successful completion of minimum educational requirements for licensure including all clinical practice and comprehensive examinations.
Current CPR Certification Meets continuing education requirements for state Licensure & CPR Certification
