Powerback is revolutionizing the delivery of recovery, rehabilitation, respiratory, and wellness services nationwide. We are seeking a Physical Therapist to provide in-home outpatient rehabilitation services for older adults. This position offers a flexible schedule and the opportunity to work in various settings to help patients achieve their full potential.
Responsibilities:
- Provide outpatient rehabilitation services in a patient's private residence or in an independent living facility
- Evaluate and treat patients, make referrals to other disciplines, and develop individualized therapy programs
- Instruct and educate patients and caregivers to promote independence and productivity
- Screen, examine, evaluate, and establish diagnosis and treatment plans
- Perform routine reexaminations and modify treatment plans as needed
- Design and fabricate adaptive devices and train patients in their use
- Incorporate health and wellness programming into treatment plans
- Perform home and community mobility assessments and provide recommendations for safe mobility
Requirements:
- Valid Physical Therapy license in the state(s) of practice
- Master's degree in Physical Therapy or related field, or Bachelor's degree plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable transportation to and from patients' residences
