Shift:Tue-Fri, 7:30am-4:00pm (32 hours per week)Location:3 days per week in McKeesport, 1 day in HomesteadSummary:Community LIFE is an all-inclusive program that empowers older adults (age 55+) to remain at home, while preserving their dignity, independence, and quality of life. Our Physical Therapist works with Community LIFE participants in an effort to regain or compensate for mobility, strength, range of motion, pain, and/or discomfort with the expected outcome of helping improve the quality of life and independence. The Physical Therapist must demonstrate the knowledge and skills necessary to assess, plan care for and provide service to frail elders according to assigned responsibilities and Community LIFE standards. As the primary contact, the Physical Therapist performs utilization case management for all inpatient admissions and transfers to Skilled Nursing Facilities. The Physical Therapist directly supervises the PTA.
