Advanced Practice Provider APP Nurse Practitioner NP Physician Assistant Overview: The MGH Complicated Pancreatitis Treatment Center (CPTC) is seeking a full-time nurse practitioner or physician assistant to join its outpatient clinic. The CPTC provides comprehensive, multidisciplinary care in an integrated inpatient and outpatient model for patients with sequelae of pancreatitis (pancreatic necrosis, pseudo cysts, chronic pancreatitis, etc.). The MGH CPTC is the only program of its kind in the New England area, and likely the country, and thus is quickly becoming the predominant referral center in the region. The CPTC has recently received attention from two of the largest Pancreatitis specific advocacy groups in the country (National Pancreatitis Foundation and Mission Cure) and are being highlighted on a national scale. The role of the outpatient nurse practitioner or physician assistant (advanced practice provider or “APP”) is to function as the primary touch point for all CPTC patients in the outpatient setting. Under the supervision and direction of the Advanced Practice Provider (APP) Manager and faculty of the General GI team, and in accordance with the rules and regulations of Massachusetts governing nurse practitioner and physician assistant practice, the APP will play an important team role with care delivery and continuity for patients in the CPT C. This includes patients who have transitioned from the inpatient to outpatient settings, outpatient referrals once seen in the MDC, and all urgent visits for this patient population. Additionally, they will take a primary role for preoperative education for patients planned for total (or partial) pancreatectomy with islet auto-transplantation. In addition to clinical activity, the APP in this role will be actively involved in the CPTC program development including hospital outreach, marketing, protocol development and other projects related to the development and growth of the MGH CPT C. This role is a 5 day a week position with a mixture of in person and remote work. Principle Duties and Responsibilities: Clinical: - Will see a variety of in person, virtual/telemedicine, and urgent add-ons seeing outpatients discharged from the Inpatient CPTC service - Includes discharge telemedicine visit within 3-5 days of hospital discharge - In person or virtual appointments every 1-2 weeks for the first 3 months (approx.) depending on individual patient needs and MDC involvement - Includes patients after percutaneous drainage, endoscopic necrosectomy and surgical intervention - Management of tube feeds and titration to resumption of oral intake - Triage and management of outpatient urgent needs - Endocrine and Exocrine insufficiency management in the post-discharge period - preoperative education for patients in the Islet Auto-Transplantation program - Patients initially screened through the MDC who require surveillance and/or ongoing outpatient management - Post-Discharge Acute Pancreatitis Patients - Care Coordination amongst multidisciplinary specialties - Work collaboratively with members of the GI and CPTC team including MDs, other advanced practice providers, nurses, medical assistants, and patient service coordinators to ensure excellent patient outcomes and patient experience. - Evaluate patients, order tests, procedures, and imaging, and follow up on results and document details in the Epic medical record system.
