When you join the Visiting Nurse and Hospice for VT & NH, you'll become part of a dedicated team delivering outstanding home health and hospice services that enrich the lives of the people we serve. *Physical Therapist Assistants* at VNH, under the supervision of a registered Physical Therapist and in accordance with agency policies and state practice, act as a member of the rehabilitation staff, providing physical therapy services according to a patient plan of care. *The PTA will:* * Provide treatment to patients as prescribed by physician and after initial and periodic evaluation of patient's physical therapy and rehabilitation needs have been established by a RPT. When PTA is going to be following a new patient, the RPT will make the initial evaluation visit and assign care to the PTA as appropriate. * Treat patients within plan of care developed with RPT to relieve pain, develop or restore function, and maintain maximum performance using physical means, such as exercise, massage, heat, water, light and electricity. * Direct and aid patients in active and passive exercises, muscle re-education, gait and functional training, activities of daily living including transfer activities and prosthetic training. * Make use of equipment such as ultra-violet and infrared lamps, low voltage generation, diathermy and ultrasonic machines. * Observe, record, and reports to the RPT the patient's reaction to treatment and change in patient's condition. * Teach the patient and caregivers self-care techniques and activities as appropriate. * Arrange with health agency for use of equipment that is available to patients. * Complete and submit accurate and relevant clinical notes and required documentation in a timely manner in accordance with expected agency standards. * Report to RPT any change in patient condition. * Possess thorough knowledge of principles and practices of physical therapy. * Be knowledgeable of socioeconomic factors affecting patients and families. * Be knowledgeable of and ability to apply community health principles and practices. * Have the ability to learn to work with patients' families and community environment. * Support practices and communicates VNH's mission and values * Interact with clients, referral sources, co-workers and others in a positive, consumer-oriented manner. * Adhere to the Agency's organizational ethics and corporate compliance plan and safety program *Minimum Qualifications:* * A.S. degree from an accredited school for physical therapy assistants. * Two (2) years acute care experience preferred. * Comprehensive knowledge of the principles, concepts and methodology required of a physical therapist and its practical application in the treatment of patients at home. * PTA license in VT and NH We would love to meet you and tell you more about this engaging and fulfilling part of our healthcare delivery system.
