Assist the PT in the over direct of the physical therapy program.SpecificationsEducation: Associates Degree in Applied Science of Physical Therapist AssistingExperience: 1 year of experience working with the frail elderly population.Number and Type of Employees Supervised (optional): NoneLicensure, Registry or Certification Required: Licensed Physical Therapist Assistant in NCSpecial Training: Demonstrates clinical competency with geriatric population. Only act within the scope of his or her authority to practice. Meet a standardized set of competencies for the specific position description established by Senior TLC and approved by CMS before working independently.Immunizations: Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.Ages of Patients Rendered Care: Adult Geriatric Key Responsibilities: (*denotes an age-related skill or task)
  • Assists Physical Therapist in communicating participant changes to team members.
  • Assists Physical Therapist in implementation of treatment program as established by Physical Therapist.
  • Records and signs treatments in participant's chart, records signs and dates Progress notes.
  • Assists Physical Therapist in planning and development of maintenance, exercise and therapy programs for participants.
  • Actively functions as a member of Multidisciplinary Team and participates in case conferences if needed under direction of a Physical Therapist.
  • Assists with the coordination of 24-hour care delivery by providing training to family members, friends, and STLC staff members for those parts of the treatment plan that can be carried out by them as directed by the Physical Therapist.
  • Provides instruction and guidance to program aides and other staff who are implementing a Physical Therapy treatment program.
  • Other duties as assigned by supervisor.
Participant Interface: Must be comfortable and eager to engage our participants in a warm and welcoming manner while maintaining a professional distance and attitude. Experience with issues of aging and health challenges would be helpful. Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
