- Assists Physical Therapist in communicating participant changes to team members.
- Assists Physical Therapist in implementation of treatment program as established by Physical Therapist.
- Records and signs treatments in participant's chart, records signs and dates Progress notes.
- Assists Physical Therapist in planning and development of maintenance, exercise and therapy programs for participants.
- Actively functions as a member of Multidisciplinary Team and participates in case conferences if needed under direction of a Physical Therapist.
- Assists with the coordination of 24-hour care delivery by providing training to family members, friends, and STLC staff members for those parts of the treatment plan that can be carried out by them as directed by the Physical Therapist.
- Provides instruction and guidance to program aides and other staff who are implementing a Physical Therapy treatment program.
- Other duties as assigned by supervisor.
Participant Interface: Must be comfortable and eager to engage our participants in a warm and welcoming manner while maintaining a professional distance and attitude. Experience with issues of aging and health challenges would be helpful. Demonstrates knowledge of and supports PACE mission, vision, values, standards, policies and procedures, operating instructions, confidentiality standards, and the code of ethical behavior.
