JOB SUMMARY: *Encouraging New Graduates to Apply!* Responsible for provision of physical therapy treatment and carrying out the plan of care as directed by the physical therapist and referral from the physician in any and all areas served. The treatment will be documented for communication with the resident's M.D. and all members of the multidisciplinary care team in the form of treatment notes. Physical Therapist Assistants (PTA) will work weekends and holidays as needed, according to a posted schedule.II. PRIMARY JOB DUTIES:Monitors resident's response to treatment and adjusts the treatment intervention as needed. Prepares and maintains all documentation as required.Discuss the resident's progress with the attending physician, family, nursing and other allied health professionals as needed.Assists in discharge planning and instructs resident and family in a home program and documents teaching.Has adequate knowledge of MDS, PRI requirementsPerforms quarterly, annual & significant change screens as dictated by the Physical Therapist.Assists and orientates new employees in departmental forms and procedures. Maintains work environment in a manor conductive to safety.Performs other related duties as requiredSelects appropriate environment/equipmentPositions & drapes patient for maximum comfort, safety and effectiveness.Reacts to change in resident's status in a given treatment session.Administers chosen treatment according to established guidelinesPrepares individual care plans specific to each resident and modifies these as neededOther duties as assignedWeekend and holiday rotation as neededIII. JOB SPECIFICATIONS:Education: Associate Degree: Physical Therapist AssistantLicense/Certification: Current N.Y.S. Physical Therapist Assistant CertificationVI.
