- Full Time position open for Family Medicine, Internal Medicine or IM/Peds Physician, BC/BE, MD/DO
- Patterson Avenue FP is a Primary Care First Practice - a CMS value-based care model
- Practice hours are 8a-5p M-F
- 36 patient contact hours and 4 hours admin per week
- Call is 1:12 days - on call once in every 12 days - shared by all providers, Rx on call at provider's discretion, No scheduled drugs or pain meds
- No Hospital responsibility - 100% outpatient practice
- Telehealth and e-visits in addition to in-person care
- Average 18 - 20 patients per day
- EPIC EMR
- We see all ages - 15% Peds, 50% adult, 35% Geriatric
- X-Ray and Lab in practice
- Basic in-office procedures
- Dedicated 1:1 CMA/LPN support
- LCSW, RN Care Manager, Community Health Worker, PharmD and Phlebotomist in practice
- Bon Secours Radiology, Rheumatology and Physical Therapy also in the building
