The position would be full time working 40 hours per week.SummaryUnder indirect supervision, provides and manages health care for a specified patient population.Essential Functions
- Provides and manages direct patient care, including physical examinations, evaluations, assessments, diagnoses and treatment for a specified patient population.
- Engages patients and their families or caregivers in understanding, setting and monitoring self-management care plans, and documents individualized care plans and care coordination in the patient chart.
- Analyze patients medical history, medication allergies, physical condition and examination to determine best care
- Diagnose medical condition
- Prescribes pharmaceuticals, other medications, and treatment regimens as appropriate to assessed medical conditions.
- Refers patients to specialists or to internal medicine as appropriate.
- Directs and coordinates the patient care activities of nursing and support staff as required.
- Respect and promote patient rights and privacy.
- Responding appropriately to emergency situations.
- Participate in meetings to evaluate practice processes and work flows pertaining to patient care and quality improvement efforts.
- Participate in specified health promotion, education and/or prevention programs.
- Participates in population health activities as directed by the practice.
- Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards
Skills and Knowledge
- Knowledge of legal and ethical standards for the delivery of medical care.
- Understanding of relevant drugs and non-pharmaceutical patient care aids and ability to prescribe dosage and instruct patients in correct usage.
- Knowledge of current principles, methods and procedures for the delivery of medical evaluations, diagnosis and treatments in area of expertise.
- Demonstrated skill in interviewing and patient coaching.
- EHR documentation as it pertains to patients and populations of patients.
Education and Experience
- Medical Doctor with 3 to 5 years of direct patient experience which may include residency.
- Medical License, Board certification, Federal DEA certification
- Need to be Board eligible or Board Certified
