General Purpose: The Physician is expected to work closely with other members of the Health Care Team to provide quality health care to the patient in their respective discipline. Description of Duties:
- Assess health status of patient through obtaining medical history, performing a physical examination of patient and utilizing diagnostic tests.
- Develop a comprehensive care plan for each patient using Standards of Care.
- Provide in-patient care at appropriate hospital as required.
- Provide health care for acute, chronic and emergency health conditions.
- Prescribe medications as appropriate.
- Integrate health care by working with other providers and health specialists.
- Provide patient counseling and health education to patients.
- Maintain professional liaison with peers and professional organizations through continuing medical education programs, publications, membership, etc.
- Participate in the Health Disparities Collaborative and assist in reaching its goals. Ensure full commitment to the Health Disparities Collaborative's Chronic Care Model.
- Precept a Mid-Level provider, if necessary.
- Participates in QI and QARR activities, including chart reviews, as required.
- Complete all patient medical forms including disability, medical clearance, etc. on a timely basis.
- Complete all required documentation and progress notes in patient medical records by the close of the business day.
- Ensure that encounter forms are complete at the end of each visit; ensures that CPT, ICD-9 and signatures are present on each encounter and that all services provided are appropriately documented on the encounter.
- Submit out-patient encounters on a daily basis and in-patient encounters on a weekly basis.
- Attend and participate in monthly department, All Staff and other required meetings.
- Be familiar with and adhere to all clinical, administrative, and general policies and procedures as outlined in the Policy and Procedure Manual and Employee Handbook.
- Maintain confidentiality of all aspects including, but not limited to, patient confidentiality, financials, and employee relations.
- Perform other duties as assigned.
Education Required:
- Graduation from an accredited medical school with a New York State license to practice medicine
- Must be Board eligible with the expectation that will become Board Certified in given specialty within five (5) years of hire

- Work environment: While performing the duties of this job, the employee is exposed to climate and noise conditions prevalent at the time as well as virus, disease and infection relative to patient contact and specific job duties. The noise level in the work environment is usually minimal, and includes but is not limited to voices, office machines and cleaning equipment.
