Sun River Health is looking to hire an accomplished full-time Primary Care Physician (IM/FM)/Infectious Disease Specialist to join our team in the Bronx at our Inwood Health Center. . The Primary Care Physician is responsible for managing a diverse patient panel with multiple co-morbidities in a busy Federally Qualified Health Center. Care is delivered in collaboration with advance practice providers, on-site support staff, with an emphasis on preventative health and quality patient care. Will treat primary care and cardiology patients. Essential duties and responsibilities for a Primary Care Physician:
- Participate in in planning and developing health care services, treatment of conditions and illnesses within scope.
- Diagnose and provide treatment plans for patients.
- Collaborate and supervise advanced provider clinicians.
- Complete and participate in chart review process.
- Referral and follow-up of patients when needed.
- Collaborate with other members of the Health Center in order to maintain high quality standards and achieve client satisfaction.
Minimum Education Requirement:
- Doctorate Degree (MD, DO)
Licensure/Certification/Registration:
- Active/Valid license as a MD or DO in New York State, DEA, Board certification
Specialized Skills & Knowledge:
- Bilingual in English and Spanish (preferred)
- Experience in HIV care and MAT (Medication Assisted Treatment) (preferred)
- Demonstrated ability to work with diverse patient population in community health setting.
