Reporting to the Director of Provider Relations, the Sr. Physician Liaison is responsible for:

  • Creating and executing on community outreach strategy for assigned markets
  • Performing outreach to PCPs, Hospitals, SNFs as assigned.
  • Cultivating collaborative relationships with community providers.
  • Driving positive relationships with key accounts to increase number of referrals/endorsements received from community providers.
  • Timely and accurate documentation of outreach to community providers in Salesforce.
What you will do:
  • Develop network provider and facility engagement strategies
  • Conduct outreach for new business opportunities both telephonically and in-person
  • Engage patients via telephonic outreach and in-person canvassing
  • Engage with Health Systems, IPAs, ACOs, Physician Groups & PCPs and other providers to build collaborative relationships
  • Educate provider offices, health systems, and patients on ConcertoCare products and services
  • Utilize reporting and KPIs to track activity and key network information
  • Continually identify and implement opportunities to improve network provider engagement defined by collaboration with ConcertoCare clinical teams and patient referrals into ConcertoCare clinical programs
  • Improve network facility (e.g., hospital, SNF) engagement as defined by ConcertoCare clinician access (in person and / or telephonic) to attributed patients and opportunities for inpatient and discharge care collaboration
What you need:
  • Bachelor's degree preferred
  • 5+ Years Provider Network Management /Provider Engagement Experience
  • Health Plan relationship experience and knowledge of value based care is a plus
  • Provider and/or Medicare Advantage health plan experience
  • 2+ years' experience using a CRM (preferably Salesforce)
  • Advanced analytical skills
  • Excellent verbal and written communication skills
  • Ability to travel within the east coast of United States, up to 50 % of the time
  • Ability and means to travel as needed in a timely manner to locations that may have limited access to public transportation; proof of liability and property damage insurance on vehicle used is required. DRIVER REQUIREMENTS: Licensed for a minimum of 3 years; No undisclosed violations and/or accidents within 3 years; No vehicle related suspensions/reinstatements; No DUI, reckless driving or felony conviction within the last 7 years.
COVID-19 Vaccination Policy Our COVID-19 Vaccination Policy is aligned with the CMS Omnibus COVID-19 Health Care Staff Vaccination Rule that mandates providers who work in Medicare and Medicaid certified facilities to be fully vaccinated to best care for our frail and elderly populations. Based on this CMS requirement, ConcertoCare requires all frontline workers to be fully vaccinated. Base Salary/ Wage Range $50,000 - $57,000 plus annual bonus. Compensation for the role is commensurate with the candidate's qualifications, skills, competencies, and experience and may fall outside of the range shown.
