**Job Title:** Physician **Job Specialty:** Geriatric Medicine, Internal Medicine, or Family Medicine **Job Duration:** Permanent **Shift:** Days 8:00 am-5:00 pm **Guaranteed Hours:** Not specified **Experience:** Minimum of 1 year working with the frail or elderly population **License:** Active Delaware license **Certifications:** BLS, Board Certified in Internal Medicine or Family Medicine, CSR/CDS Certification, DEA Registration **Must-Have:** Interest in permanent position, EMR TruChart experience highly preferred, clean/clear malpractice record highly preferred **Job Description:** - Provide primary medical care to participants, including assessments and development of care plans in collaboration with the interdisciplinary team.- Function in a collegial relationship with other healthcare professionals, making independent decisions regarding medical needs and health regimens.- Perform medical procedures as indicated.- Participate as a member of the interdisciplinary team in assessments and attend daily updates.- Provide and implement care plans, evaluating their effectiveness.- Integrate the primary care treatment plan with the overall care plan developed by the interdisciplinary team.- Interact with team members to meet emergent and acute needs of participants.- Evaluate and treat participants during acute illness.- Refer participants to medical specialists as needed.- Manage all care of participants in nursing homes in collaboration with the interdisciplinary team.- Maintain communication with nursing home staff for continuous monitoring of participants.- Admit participants to hospitals when necessary and oversee their use of medical specialists and inpatient care.- Complete documentation and use appropriate diagnostic coding.- Review medical outcomes, statistical data, and utilization data, working to establish best practices and improve participant health.- Provide regular visits to manage the care of participants in nursing homes.- Communicate and follow up with specialists regarding the outcomes of visits.**Compensation and Benefits:**- Competitive compensation and benefit package- Supportive and motivated colleagues- Patient-centered focus- Health, dental, and vision insurance- Short and long-term disability- Retirement plan- Generous paid time off- Mileage reimbursement- Comprehensive orientation- Opportunity for personal growth and professional advancement
