- Monday through Friday, 8:00 am to 4:30 pm
- Hospital-Based Program
- Required Procedures: Conventional Radiotherapy, 3- dimensionalRadiotherapy, External Beam Radiation - 3- dimensional -chemoradiotherapy, External Beam Radiation - Image-guided radiation therapy, External Beam Radiation - Intensity-modulated radiation therapy, and Stereotactic Body Radiation Therapy
- Credentialing Timeframe: 60 to 90 days
