WHO WE ARE:Community Healthcare Network (CHN) is a not-for-profit organization providing more than 65,000 New Yorkers with primary and behavioral healthcare, dental, nutrition, wellness, and needed support services.Our network is made up of 14 federally qualified health centers throughout Brooklyn, the Bronx, Queens, and Manhattan, along with a fleet of mobile vans that bring health services to underserved people in need throughout New York City. We turn no one away.POSITION SUMMARY:Responsible for conducting outreach, providing general risk reduction, providing referrals to needed services, distributing HIV/AIDS information and condoms, participating in training and support activities, utilizing data forms to document and report activities performed. DUTIES AND RESPONSIBILITIES:Essential Functions:
  • Compliance with Employee Health Services.
  • Conducts community outreach and door to door outreach in targeted areas identified by the Program Manager to meet grant targets.
  • Participates in community health and community fairs as an agency representative.
  • Provides referrals of clients to necessary health and support services in the community, including making appointments for clients at CHN clinic sites.
  • Performs other related duties, as assigned.
  • Identifies community resources and makes contact with such agencies.
  • Escorts clients to necessary services, as appropriate.
  • Documents clients reached through community outreach and report this to the program manager.
  • Distributes HIV/AIDS educational literature and condoms to clients and agencies.
  • Participates in training, in-services, and support activities for peer educators.
  • Facilitates related services for health center patients and CHN staff as appropriate, with respect to their confidentiality and privacy.
  • Identifies community resources and makes contact with other agencies.
  • Escorts clients to necessary services, if appropriate.
  • Documents clients that were reached through street outreach.
  • Distributes HIV/AIDS education materials and condoms to clients and other agencies.
  • Assists in CHN's clinic outreach efforts to targeted populations.
  • Performs other related duties as assigned
  • EDUCATION & EXPERIENCE REQUIRED:
    • High School diploma or G.E.D. preferred with some experience in community work.
    KNOWLEDGE, SKILLS AND ABILITIES REQUIRED:
    • Should be a member of the targeted communities and of the targeted service groups.
    • Have good communication skills and experience with HIV issues.
    • Excellent interpersonal skills.
    • Demonstrates ability to work with HIV-infected individuals.
    • Knowledge of geographic area of work site and of language appropriate to community.
    • Interest in and conviction of purpose and aims for all programs in the clinic.
    • Respect for the rights of individuals to make their own decisions about their medical care.
    • Ability to communicate easily and display a cordial manner towards individuals from a variety of socio-economic, cultural and religious background.
