Position Details: We are seeking a dedicated and experienced Pod Leader to oversee a team of Licensed Practical Nurses (LPNs) responsible for reviewing patient charts and managing patient care. The Pod Leader will play a crucial role in ensuring the accuracy and completeness of patient records, enhancing patient outcomes through effective case management, and fostering a collaborative environment within the healthcare team. Key Responsibilities: Team Management: Supervise, mentor, and support a team of LPNs. Provide guidance and leadership to ensure high-quality patient care and accurate documentation. Provide education for assigned care coordinators when necessary. Patient Care Coordination: Conduct comprehensive assessments of patients' medical, social, and emotional needs. Case Management: Monitor and evaluate patient progress, making necessary adjustments to care plans. Collaborate with interdisciplinary teams to optimize patient outcomes and address any gaps in care. Documentation Oversight: Ensure the accuracy and completeness of patient electronic medical records (EMR). Review patient records to confirm that they reflect the conditions and treatment requirements of each patient. Reporting and Improvement: Complete and review reports to identify and address patient care gaps and reduce hospitalizations. Implement strategies to enhance care delivery and patient satisfaction. Qualifications: Licensure: Valid Registered Nurse (RN) license required. Experience: Minimum of 2 years of experience in an acute care or managed care setting preferred. Experience with Geriatric behavioral health is a plus. Skills: Proficient in conducting patient assessments, monitoring vital signs, and utilizing EMR systems. Excellent communication and interpersonal skills are essential for effective collaboration with healthcare professionals. Attributes: Responsible, patient, and capable of leading a team with a focus on delivering high-quality patient care and maintaining accurate records.
