HOURLY RANGE: $50.137 - $67.680DISTINGUISHING FEATURES OF THE CLASS: The work involves analyzing clinical data for various health populations and utilizing clinical knowledge to develop methods for improving health status by population. The incumbent utilizes the data in electronic medical records systems for understanding the needs of specific health populations and leverages such data for developing methods to improve type of care provided. The incumbent provides data-based and clinically-focused recommendations and methods to physicians and administrators involved in improving overall patient group health statuses. Work is performed under the general supervision of the Director of Outpatient Operations - Primary Care. Supervision is exercised over lower-level nursing, paraprofessional and clerical staff, and lower-level staff receiving training. Does related work as required.TYPICAL WORK ACTIVITIES:Analyzes and classifies group population health clinical and quality outcome data;Prepares findings and develops recommendations utilizing clinical expertise on quality improvement and applicable methods used to improve health status by specific population (e.g. hypertension, diabetes);Designs data structures within electronic medical record systems for identifying richer data sources and optimizing reporting abilities;Creates and amends workflows utilizing electronic medical records systems to provide care providers with structured criteria for assessing patient health and managing complex, chronic health conditions by specific population;Provides clinical expertise to physicians and administrative staff on population health management initiatives;Assists in the evaluation of quality and effectiveness of application hardware/software and system performance;Coordinates and trains staff on changes to electronic medical records systems and related protocols for assessing and documenting patient health status;Collaborates with Health Information Management on developing and implementing hardware and software changes to electronic medical records systems;Assists in the development and implementation of policies and procedures related to population health management electronic data initiatives;Performs professional nursing services as required and responds to emergency situations.FULL PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of the principles, practices and terminology associated with population health; thorough knowledge of the principles and practices associated with professional nursing; thorough knowledge of specific health populations and related diseases; thorough knowledge of the electronic medical records systems utilized at ECMCC; ability to analyze data and classify patient health status; ability to develop and communicate clinical recommendations for improving health care provided to specific health populations; ability to design data structures within an electronic medical record system; ability to develop protocols for questioning patients and identifying their symptoms and health status; ability to train staff on proper utilization of electronic medical records systems and related protocols; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with a diverse constituency; ability to utilize a variety of electronic software applications; sound professional judgment; capable of performing the essential functions of the position with or without reasonable accommodation.MINIMUM QUALIFICATIONS:
  • Possession of a Master's Degree and two (2) years of professional nursing experience in an ambulatory services* setting; or:
  • Possession of a Bachelor's Degree and four (4) years of professional nursing experience, of which two (2) years were in an ambulatory services* setting; or:
  • An equivalent combination of training and experience as defined by the limits of (A) and (B).
SPECIAL REQUIREMENTS:Possession of a license and current registration to practice Nursing in New York State at time of appointment and maintenance throughout duration of appointment.*NOTE 1: Ambulatory services is defined, for qualifying purposes, as medical care in which patients are admitted, treated and discharged all on an outpatient basis.
