Population Health Director -- Woburn, MA
- This position provides strategic planning, development, implementation and leadership oversight for Marquis Health System's Care Management facility teams.
- The Population Health Director is considered a subject matter expert for the integration of system-wide Population Health initiatives.
- Assures the implementation of consistent care management practices including role functions, competencies and performance management.
- Provides oversite, Support and coordination of care within the Skilled Nursing Care Management departments, Utilization Review functions and teams, the facility Case Management department.
- Works closely with the Care Management teams across the health system within long term care, home care, rehabilitation and behavioral health. Works in partnership with other Care Management leaders in Complex and Chronic Care, Population Health Analytics and Patient Throughput.
- Collaborates with corporate and health system team members to ensure that services are delivered within a culture that strives for excellence respect, ad continuous improvement and reflects Marquis Health System's Mission, Vision and Core Values.
Roles & Responsibilities
- Develops structures and processes that contribute to continuity of care across the organization
- Works collaboratively with internal and external resources and agencies, physicians and all members of the team in meeting customer needs and organizational goals
- Readmission data; Care Management dashboard; Care; supervisor observation.
- Consistently creates and implements programs and processes that support seamless, patient-centric, best practice continuity of care in between multiple care settings.
- Programs are clinically integrated, incorporates the Voice of the Internal and External customers, ensures metrics that are Critical to Quality are part of the final product and are sustainable.
- Successful demonstration of program/project management and data analysis skills in population health management. Programs and processes are consistent with change management and process improvement principles. Business partners are kept abreast of departmental programs and processes.
- Development, implementation and oversight of integrated programs and processes that support transformation to a value based, consumer centric health care environment.
- Works closely with Organizational partnering departments supporting population health analytics, population health IT, finance and payer contracting to assure alignment of care management practices.
- Creates a professional, productive, and efficient work environment.
- Demonstrates ability to lead large scale transformational change in health care at the organizational/system level and work effectively with all levels in the organization, individual and team, across disciplines and geography.

- Develop and maintain a communication infrastructure to support knowledge sharing, project updates, and process improvement initiatives and programs regarding Care Management / Population Health initiatives. Keeps key stakeholders, from executives to team members informed. Develops and reports on key metrics. Interfaces with managed care and other external organizations as relevant.
