HOURLY RANGE: $34.44 - $47.10DISTINGUISHING FEATURES OF THE CLASS: The work involves supervising and coordinating the delivery of comprehensive discharge planning services aimed at improving health outcomes, reducing health inequities and enhancing overall well-being to patients in the PopulationHealth department at the Erie County Medical Center Corporation (ECMCC). The incumbent develops and oversees a coordinated approach to discharge planning, including admissions, placement, transfer in, transfer out, discharge, and post-care follow-up aimed at improving the health outcomes for patients experiencing disparate health impacts. Work is performed under the general supervision of the Assistant Director of Population Health or other higher-level administrator. The scope of this specification differs from that of traditional Discharge Planning due to its focus on patients experiencing health disparities and requiring services for such populations. Supervision is exercised over lower-level Population Health professional and support staff. Does related work as required.TYPICAL WORK ACTIVITIES:Supervises discharge planning services and the staff providing such services as related to Population Health;Determines work assignments, evaluates and disciplines, monitors time and attendance, etc. for lower-level staff;Coordinates discharge planning services with applicable clinical and non-clinical staff as part of a comprehensive care management team within Population Health focused on improving health outcomes for patients with health inequities;Assists with educating patients and their families who are experiencing health inequities about their diagnosis, medications, and care plans in collaboration with clinical care team;Manages transfer operations for patients entering, leaving or transferring within the Population Health department to/from other departments within ECMCC;Collaborates with leadership on existing and new public health programs to improve health outcomes for patients experiencing disparate health impacts;Assists with tracking patient outcomes, readmission rates, and discharge-related metrics to assess the effectiveness of interventions and support quality improvement initiatives;Partners with community-based organizations to develop and lead an ongoing education series on Population Health, Health Equity and health care;Assists in the development and implementation of policies, procedures and workflows for patient throughput and related activities such as admission, transfer, discharge and full bed capacity within Population Health department;Communicates and educates staff on applicable community resources, third-party payers, regulatory requirements and accreditation standards related to Population Health and Health Equity;Serves as a subject matter expert and assists in planning, developing, and implementing a range of Population Health and Health Equity initiatives and quality improvement projects across ECMCC;Attends and may organize various meetings, events, committees, conferences, trainings, etc.FULL PERFORMANCE KNOWLEDGES, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS: Thorough knowledge of the principles and practices of discharge planning as related to population health and patient's experiencing health inequity in an ambulatory care setting; thorough knowledge of applicable laws, rules, regulations and ECMCC accreditation standards as they relate to discharge planning, population health, and patient throughput services in an ambulatory care setting; thorough knowledge of ECMCC and its population health services; thorough knowledge of available community resources; ability to comprehend, evaluate and develop corrective actions to discharge planning, population health, and patient throughput issues; ability to match patient discharge needs with available community services; ability to interpret data and prepare reports utilizing such data; ability to plan the work and evaluate the performance of lower-level staff; ability to communicate effectively, both orally and in writing; ability to establish and maintain effective working relationships with a diverse constituency; ability to utilize a variety of electronic software applications; sound professional judgment; capable of performing the essential functions of the position with or without reasonable accommodationMINIMUM QUALIFICATIONS:* Possession of a Master's degree* in public health, social work, psychology, business administration or closely related field and two (2) years of experience in public health or discharge planning, six (6) months of which was in a health care setting; or:* Possession of a Bachelor's degree* in public health, social work, psychology, business administration or closely related field and four (4) years of experience in public health or discharge planning, six (6) months of which was in a health care setting; or:* An equivalent combination of training and experience as defined by the limits of (A) and (B).NOTE*: Your degree must have been awarded by a college or university accredited by a regional, national, or specialized agency recognized as an accrediting agency by the U.S. Department of Education/U.S. Secretary of Education.
