Nurse Practitioner or Physician Assistant opportunity in Post Acute Rehabilitation in Monterrey County Daily morning Rounds and Medical Management in top-ranked facilities. Supportive team! NO nights, no long days, no weekends.About the Nurse Practitioner or Physician Assistant opportunity in post-acute carePhysician owned and lead company (you get to interact with the leadership)Young company with strong peer support and growth strategy (no corporate mandates or tedious process)Team is MD, NP/PA, and Medical CoordinatorDaily patient schedule is 15-18 but census varies3-5 Admissions a day (schedule adjusted to allow time and most other appts are follow ups on busy admission days)See patients for acute visits, order labs and diagnostics when needed, document progress notes, attend interdisciplinary meetings, communicate with patients, staff, and family.Monthly follow up for Long-Term Care patientsEMR is GehrimedMedical Coordinator also at facility and helps with schedule, notes, lab and diagnostic reporting, (think Scribe/Support)Rounds between 7-9 am each dayAvailable on company phone until 5 each dayLocal short distance travel only.Requirements of the Nurse Practitioner or Physician Assistant opportunity in post-acute careMaster's Degree with clinical focus on Primary Care and Geriatric MedicinePhysician Assistant or Nurse Practitioner Medical License and experience with managing complex patients in hospital or office settingNational CertificationCalifornia State Medical LicenseCalifornia DEAInterest in managing chronic medical conditions like Hypertension, Diabetes, CVA, CHF, and DementiaStrong EMR experience as timely documentation is requiredTop candidates will have Critical Care, Geriatrics, Hospitalist, Home Health, or Internal Medicine experiencePerks of the Nurse Practitioner or Physician Assistant opportunity in post-acute careBase salary range is $150,000-$170,000Monthly bonus 5% at year one, 7.5 % over year two, and 10 % at year threeHealth Insurance (Indvidual and Family)401K with matchDental and vision insuranceCME allowance $1500 and 2 Professional daysPTO- 18 days8 National Holidays offPhone reimbursement each monthMalpractice with Tail coverageLicense expenses coveredNo weekends or eveningsBe part of a tight knit medical team that focuses on quality over quantity.
