Job Description:

For credentialed applicants, we are currently offering a $3000.00 sign-on bonus for candidates hired for any of our four locations with a current FCB certification (Child Protective Investigator, Child Welfare Case Manager, Child Welfare Licensing Counselor) for this position.

POSITION SUMMARY:

This position is responsible for assisting adoptive families with information and support services. This person will provide case management crisis intervention, advocate through interagency programs, hold monthly support groups, seek education training for families and providers to become adoption competent, provide yearly follow up calls with adoptive families, and assess and complete subsidy increase requests. Additionally, this person will process out-going ICAMA requests for post-adoptive families moving out of state.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

* Work closely with other Adoption unit staff and the Clinical Services Department at CPC to preserve adoption placements.

* Become the Single Point of Access for adoptive families seeking information about post adoption services.

* Update adoption information on agency website, community connector, and other forms of communication.

* Refer families to various agencies within the community who are adoption competent that accept Medicaid for psychological evaluations, psychiatric evaluations, behavioral analyst, counseling, medical care, and other needs that may arise. Work with adoption competent therapists to provide short term crisis stabilization for families.

* Develop techniques and programs for the post adoption services i.e. a list of therapist that deal with pre/post adoption.

* Create a monthly email newsletter for families discussing training opportunities, support group minutes, and useful resources.

* Maintain a spreadsheet of all adoptive families and updated contact information.

* Complete yearly contact with adoptive families per DCF and document contact in FSFN.

* Seek and update educational and training resources for adoptive families. Notify adoptive families of conferences including professional training and educational tools within the community. Seek school professionals knowledgeable and skilled in the area of adoption issues.

* Attend and participate in Family Service Planning Team meetings related to the children that are considered a 'community' child but has been adopted through the State of Florida.

* Actively participate in Integrated Practice Team wit CPC Prevention staff and DCF for families seeking resources or assistance with post adoptive placement stabilization

* Provide case management for post adopt families. Keep a tracking log of contacts and services provided to families and document efforts on the FSFN case. Provide case management for post adopt families and documenting services provided to families in the post adoption tab on FSFN.

* Crisis Intervention that may require home visits to the adoptive placement and needed referrals through the Clinical Services Department and/or Prevention Program.

* Review post-adoption services funding requests to ensure that all requirements for eligibility have been met. Essentially, this includes all primary funding sources (Medicaid, health insurance, etc.) have been exhausted, the service is not already included in the monthly medical/supplemental subsidy, ensure proper consultations have been made with TCM, SAMH, SSA, etc., service is reasonable and customary, the child remains in the care of the adoptive family, and ensure that the service is reasonably expected to preserve the family or promote safety of children in the family.

* Advocate through interagency programs to include, but not limited to, Educational issues, Placement in residential facilities, Other financial assistance for services not covered by Medicaid or private health insurance, and Medicaid covered services which may be inaccessible in their community (i.e. dental, vision, etc.)

* Partner with Medicaid providers to share updated information and reach out to families who may be at risk.

* Follow up with all intakes received by the hotline that pertain to post adoption cases. Email the assigned CPI to determine if Post Adoption Services can be involved to provide support to the family.

* Process all out-going ICAMA requests for post adoptive families moving out of state.

* Track number of families served every month and report to Karin Flositz and supervisor before the 5th of the following month.

* Complete tuition waiver forms for clients and complete redactions for families or previously adopted youth requesting to see their file.

* Participate in the quality improvement process.

EXPERIENCE/PERFORMANCE REQUIREMENTS (Knowledge, skills and abilities):

* Current Child Welfare Credentials

* Three or more years working in Case Management

* Ability to work independently

* Effective verbal and written communication skills

* Good organizational skills

* Strong interpersonal and communication skills

* Ability to maintain confidentiality of sensitive data

* Ability to work flexible hours

* Two or more years working in Adoptions, preferred

EDUCATION REQUIREMENTS REQUIRED:

* Bachelor's Degree from an accredited college or university in a human services related field.

LICENSES AND CERTIFICATIONS:

* Possess a current Background Clearance Screening Letter as required by DCF; and successfully complete the background screening requirements for Community Partnership for Children.

* If local travel is required, a Valid Florida driver's license and documentation of current automobile insurance, with $100,000/$300,000 bodily injury liability limit, is required

PHYSICAL DEMANDS AND WORK ENVIRONMENT:

Employee will work in an office/clerical environment. The employee will work predominantly seated with recurrent need to walk, stand, and bend from the waist. Occasional light lifting, stooping, and climbing may be required. Occasional local travel.
