This position is eligible for a $5,000 sign-on bonus Summary Works independently to perform Medicare Annual Wellness Visits and facilitates advance care planning conversations using the evidenced based Respecting Choices model. Accountable for assuring delivery of quality nursing care for the designated patients of PPG in accordance with the individual patient's needs, Medicare Wellness Clinic standing orders, physicians'/advanced practice providers' orders, PPG and PH policies, and standards of practice. Responsible for working collaboratively with leadership, providers, and co-workers to provide excellent service to our patients. Education Must be a graduate of a professional school of nursing and hold an ASN or BSN. Must complete Parkview New Hire Orientation, Patient Care Orientation and PPG orientation as provided and required. Must complete the provided Advance Care Planning First Steps facilitator training within one year of hire. Licensure/Certification Valid Indiana Registered Nurse license required. Must have current CPR certification. Experience Must have sufficient previous clinical experience. Preferred areas of experience may include; geriatric care, ambulatory care, or nurse coaching. Other Qualifications Good verbal and written communication skills required. Medical terminology and pharmacology background needed. Essential computer skills and knowledge of electronic medical records recommended. Motivational interviewing skills preferred. Why Parkview?
