Your expertise in critical thinking and interdisciplinary collaboration will be essential in ensuring effective care coordination across various healthcare settings, ultimately preventing unnecessary emergency visits, hospitalizations, and child protective services involvement.Daily Activities:A typical day will blend in-person interactions, such as warm handoffs with care teams, and remote communications via phone, secure email, and text.
