Your expertise in critical thinking and interdisciplinary collaboration will be essential in ensuring effective care coordination across various healthcare settings, ultimately preventing unnecessary emergency visits, hospitalizations, and child protective services involvement.Daily Activities:A typical day will blend in-person interactions, such as warm handoffs with care teams, and remote communications via phone, secure email, and text. - Evidence of essential leadership, communication, education, and counseling skills.

Required Qualifications
- Master's degree in Social Work, Psychology, Sociology, Public Health, Nursing, or a related field.
- One (1) to three (3) years of experience with patient populations receiving medical care or social services.
OR
- RN with a Certified Case Manager ( CCM ) credential and one (1) to three (3) years of case management experience.
OR
- Licensed Practical Nurse ( LPN ) with five (5) to ten (10) years of relevant experience in case management.
Preferred Qualifications
- Care coordination and/or case management experience.
- Bilingual (English and Spanish fluency)
