Job Description:
The Prevention and Outreach Specialist:
  • Conduct HIV Rapid Testing, integrated STD testing, and pre and post-test counseling at EPIC locations or in the community.
  • Deliver all confirmatory test results and make appropriate referrals for follow up medical care (linkage to HIV care, PreP navigation, nPEP referral, STD treatment referral) and other services as identified.
  • Contact appropriate referrals in a timely manner regarding services according to grant specifications.
  • Maintain appropriate and required tracking of referrals (including PreP/nPeP, DOH, etc).
  • Screen and identify barriers to care, refer to other services (i.e., mental health, housing), link clients to care providers, and monitor appointments and health outcomes.
  • Develop a sustainable care plan with each client to provide and conduct appropriate interventions to all eligible individuals for DOH funded interventions. The plans of care will focus on removing barriers to care, determining benefits, and linking to appropriate provider(s).
  • Complete appropriate and timely documentation for 1628s, plus all required documentation for grant approved interventions.
  • Maintain accurate documentation of client information electronically and via client files.
  • Maintain clear and up-to-date knowledge about resources and best practices in HIV, STIs, and PreP/nPep.
  • Conduct and maintain all regulatory standards for Testing, Biohazard, mobile unit, and client interventions.
  • Maintain annual training and appropriate certifications as it relates to Empath Health, EPIC, and grant funders (CDC, DOH).
  • Identify, establish and conduct innovative outreach approaches and outreach at a variety of venues and sites throughout the assigned county.
  • Attend community meetings as requested.
  • Attend all EPIC and Empath Health staff meetings.
  • Other duties as assigned by the Prevention Team Coordinator or Director of Support Services and Program implementation.
Position Requirements•Education and/or Experience:Associate's degree (A.A.) or equivalent from two-year college or technical school; or six months to one year related experience and/or training; or equivalent combination of education and experience.
  • Certifications (i.e. CPR): 500/501 HIV Testing and Training, Rapid Testing Certification, or the ability to obtain within six months of hire.
  • 1 to 2 years' experience in HIV Services or HIV/STI Prevention Services or related health and human services field.
  • Working knowledge of Microsoft Word, Excel, and basic computer software
  • Must have reliable transportation and insurance with valid license in good standing
  • Must be able to drive a 31' mobile testing unit and have appropriate licensure and insurability.
  • Bi-lingual (Spanish) a plus.
  • Rapid Syphilis, Hepatitis C, and other STI certifications, or the ability to obtain within six months of hire.
