Job Description:

Ovation Primary Care continues to grow and we are now expanding in the Utah market! We are currently seeking a highly skilled and compassionate Primary Care Provider / Nurse Practitioner with a focus on geriatrics, hospice, and palliative care to join our primary care team. As a Primary Care Provider / Nurse Practitioner with Ovation, you will play a vital role in delivering comprehensive healthcare services to our elderly population.

Responsibilities:

  • Provide high-quality primary care services to geriatric patients, focusing on promoting healthy aging and managing chronic conditions.
  • Conduct thorough assessments, perform physical examinations, and order and interpret diagnostic tests.
  • Develop and implement individualized care plans based on patient needs, including medication management, preventive screenings, and health promotion activities.
  • Collaborate with interdisciplinary teams, including physicians, social workers, and other healthcare professionals, to ensure holistic and patient-centered care.
  • Assist in coordinating and providing hospice and palliative care services, including pain management, symptom control, and emotional support for patients and their families.
  • Educate patients and their families about disease management, treatment options, and end-of-life care decisions.

Requirements:

  • Current and unrestricted Nurse Practitioner license in Utah.
  • Minimum of 2 years of experience as a Nurse Practitioner, with a focus on geriatrics, hospice, or palliative care.
  • Strong knowledge of geriatric healthcare, chronic disease management, and end-of-life care principles.
  • Excellent clinical assessment and diagnostic skills.
  • Ability to work collaboratively in an interdisciplinary team environment.
  • Exceptional communication and interpersonal skills, with a compassionate and empathetic approach to patient care.
  • Ability to travel in the Utah Area.
