Job Summary:

Are you an empathetic and dedicated individual looking to make a difference in the lives of individuals with intellectual and developmental disabilities? Join our team as a Primary Support Professional and provide Individualized Supported Living (ISL) services in collaboration with the Program Coordinator.

As a Primary Support Professional, you'll work in the home and community with assigned clients to ensure the satisfactory implementation of their Individual Support Plan (ISP). You'll provide direct support services, coordinate daily activities, and ensure the health, safety, and cleanliness of the residential homes. Our ideal candidate has experience in the IDD field, strong communication skills, and the ability to effectively advocate for clients.

Key Responsibilities:

* Work in the home and/or community with assigned clients as designated

* Participate in Individual Support Plan meetings and RN/service monitoring visits

* Train staff on client's medical needs, personal preferences, and medication compliance

* Oversee daily residential notes, goals, and health tracking; complete required checklists

* Maintain house calendar and ensure all annual medical appointments are up to date

* Ensure adequate supplies of groceries, medication, and household needs; order as needed

* Review physical condition of home to meet health, safety, and cleanliness standards

* Ensure safety kits are present and complete; review Medication Administration Records

* Complete event reports in accordance with state guidelines

* Schedule staff or cover shifts for personnel who are absent

* Track mileage and client expenditures; ensure proper receipts and money counts

* Collaborate with Program Coordinator on follow-up, training, and documentation

* Ensure safety drills are conducted and staff are trained on special diets/equipment

* Document daily exceptions and submit to Program Coordinator

* Perform other duties as assigned

Education and/or Experience Qualifications:

* Must be 18 years of age with a high school diploma or GED

* One year of experience in the IDD field or successful completion of Missouri Quality Outcomes training

Required License/Certification (one of the following):

* Valid driver's license, proof of insurance, and reliable transportation

Additional Qualifications:

* Ability to read, interpret, and write routine reports and correspondence

* Effective communication skills with clients, families, and other stakeholders

* Ability to apply common-sense understanding and deal with standardized situations

* Proficiency in basic math operations using whole numbers, fractions, and decimals
