Job DescriptionPurpose:Provides Occupational Therapy services to residents by assisting in the assessment,treatment, program planning and implementation, related documentation, andcommunication. Functions under supervision of the Occupational Therapist.Qualifications:Ability to carry out treatment plans as designed by supervising therapistAbility to document daily care as per established guidelines.Ability to effectively communicate response to treatment intervention to residents,family, and other team members.Essential Job Functions: Responsibilities include but are not limited to the following:Judgment and QualityProvides Occupational Therapy services to residents under the direct supervisionof an Occupational Therapist.Assists the Occupational Therapist in the evaluation process in the areas of selfcare, homemaking, functional mobility and coordination. Assists in theevaluation of other areas as directed.Identifies indications for and assists in evaluations at appropriate intervals underthe direction of the Occupational Therapist.Conducts both individual and group treatment as assigned by the OccupationalTherapist and outlined in treatment plan in the areas of ADL, functional mobility,ROM, strength, coordination, cognitive/perceptual remediation and functionalmaintenance.Assists in the selection of Occupational Therapy techniques/media and in thesequence of activities, adapts techniques/media as indicated by residentperformance and following consultation with the Occupational Therapist. Nomajor modification to the treatment plan is made without prior consultation withthe Occupational Therapist.Maintains acceptable resident care ratios as determined by the OccupationalTherapist.Provides treatment in accordance with established standards of practice,department procedures and productivity standards.Consistently demonstrates sound judgment in the evaluation, planning,implementation and follow-up of resident therapy programs.Assists with and participates in Continuous Quality Improvement programs asrequired.Adheres to established confidentiality standards.Follows management direction.CommunicationParticipates in resident, family and staff education.Confers formally and informally with other team members in coordinating thetotal rehabilitation program of the resident.Maintains positive relationships and rapport with coworkers, residents, familymembers and facility personnel.Communicates orally and/or in writing with clarity and accuracy.Projects a positive and professional image at all times.Maintains communication with supervisor concerning caseload fluctuations asappropriate.Planning and OrganizationAssists the Occupational Therapist in identifying a problem list, short term goals,long term goals and treatment plan.Adheres to department and resident schedule and modifies as appropriate fortreatment regimen.Organizing work area and work in progress so that minimum delay results withinterruptions or substitutions.Coordinates treatment plan with resident, family, and other team members.DocumentationAdheres to department procedures regarding documentation of OccupationalTherapy services.Records indirect time and billable treatment times according to departmentprocedures.Documents results of resident's assessment, treatment, follow-up and terminationof services.Records skilled services provided on a daily basis, including time spent withdirect patient care.Completes weekly progress notes providing information related to patientprogress and plan modification.Consistently documents and updates therapy intervention on the nursing careplan, in accordance with established time frames.
