Your primary responsibility revolves around delivering skilled nursing care and support to individuals who may have various medical conditions, disabilities, or require post-operative care. Job Title: Home Health Nurse Job Overview: Our home health nurses are responsible for assessing, planning, implementing, and evaluating patient care in a home setting. They work closely with patients and their families to provide personalized care and ensure the patient's overall well-being. Key Responsibilities:
Patient Assessment: Conduct thorough assessment of patients' health conditions, including physical, emotional, and social aspects. Collect and analyze patient data to create an individualized care plan. We use Finale Health for Oasis and visit notes. Care Planning: Develop a comprehensive care plan that outlines the patient's treatment goals, interventions, and expected outcomes. Collaborate with other healthcare professionals, such as physicians and therapists, to coordinate care. Medication Management: Administer medications as prescribed, educate patients and family members about medication usage, and ensure compliance with medication regimens. Wound Care: Provide wound care, dressings, and other treatments as needed. Monitor the healing progress and report any complications to the healthcare team. Monitoring Vital Signs: Regularly monitor and record vital signs, such as blood pressure, heart rate, temperature, and respiratory rate. Recognize and respond to any changes in the patient's condition. Health Education: Educate patients and their families on managing their conditions, lifestyle modifications, and maintaining a safe home environment. Offer guidance on nutrition, exercise, and self-care. Catheter Care: If applicable, provide care and maintenance for indwelling catheters and urinary drainage systems. Supervise Home Health Aides performing Activities of Daily Living (ADLs): Oversight of HHA who provide patients with bathing, dressing, grooming, mobility, and other daily activities as necessary. Documentation: Maintain accurate and up-to-date patient records, including assessment findings, care plans, progress notes, and any changes in the patient's condition. Communication: Communicate regularly with the case manager and patient's primary care physician and other members of the healthcare team to provide updates on the patient's progress and collaborate on treatment plans. Emotional Support: Offer emotional support and counseling to patients and their families, especially when dealing with chronic illnesses, end-of-life care, or other challenging situations. Emergency Response: Be prepared to handle medical emergencies and initiate appropriate interventions. This may include CPR, wound management, or contacting emergency services. Infection Control: Follow strict infection control protocols to prevent the spread of infections and ensure a safe environment for the patient. Qualifications:
Education: Bachelor of Science in Nursing (BSN) or an Associate Degree in Nursing (ADN). Some positions may require advanced certifications or specialized training. Licensure: Must be a licensed registered nurse (RN) or licensed practical nurse (LPN), in the State of Texas. Experience: Prior nursing experience, especially in home health or a related field, is preferred. CPR Certification: Certification in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS) may be required.
