At Homeland, every person on our team matters and fulfills a greater purpose in the lives of our residents, patients, and clients.As a PRN Nutritionist for Homeland Center, you contribute to our culture of care and trust by ensuring our residents nutritional needs are met, contributing to their overall health and well-being.The PRN Nutritionist is responsible for completing initial, quarterly, annual and significant-change nutritional assessments on residents. Participates in care plan conferences and other clinical meetings to consult with Physicians and other members of the Interdisciplinary Team to determine diet restrictions and nutritional needs of Residents. Additionally, responsible for assessing nutritional needs, diet restrictions, and current health plans to develop and implement dietary care plans and provide nutritional counseling as needed.PRN NUTRITIONIST RESPONSIBILITIES
- Completing Nutritional Assessments
- Resident Interviews and Nutrition Education
- Tracking Significant weight losses and pressure ulcers along with follow-up notes
- Entering MDS's and Updating Care Plans
- Attending Weekly Interdisciplinary Care Plan Meeting
- Attending Resident/Family Care Plan Meetings
- Meal observations
- Communication with the Interdisciplinary Team
QUALIFICATIONS FOR PRN NUTRITIONIST
- Bachelor's Degree in Nutrition.
- Certified Dietary Manager or Registered Dietitian
- 1-3 years experience in long term care, hospital, or outpatient clinical setting preferred.
