Job Summary:
A Physical Therapy Assistant is someone who provides coordinated physical therapy to patients of all age groups under the supervision of a licensed Physical Therapist. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge.

Qualifications:
* Physical Therapy Assistant licensed per the state requirements
* Good communication skills to be able to relate professionally and positively to patients, caregivers, colleagues and physicians and strong clinical documentation skills
* Ability to read, write, and speak the English language
Essential functions:
* Treats patients as directed by the Physical Therapist.
* Records treatments administered in medical record.
* Records clinical documentation according to accepted regulatory, corporate and professional guidelines.
* Participates in weekly case conferences.
* Participates in interdisciplinary communication to ensure that patients needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.
* Provides verbal or written patient status updates to supervising PT prior to reassessment and discharge visits, and as indicated by patient response to treatment.
* Participates in in-service training programs.
* Responsible for assuring patient safety.
* Complies with state regulations and national standards as they apply to the practice of physical therapy.
* All documentation will be completed and synched by 9:00AM the following day, 7 days per week.
