Description

Organization Overview: The Goddard Riverside Community Center (Goddard Riverside) and the Stanley M. Isaacs Neighborhood Center (Isaacs Center) are well-established Manhattan community-based agencies that support services to approximately 30,000 New Yorkers. We are two of New York City's leading human services organizations meeting the needs of children, youth and families, homeless individuals, and older adults throughout Manhattan and New York City. Program Description: The Isaacs Center Aging Services are funded by Department for the Aging and The New York State Office for the Aging (SOFA). About 70% of the seniors are Spanish-speaking and 5% Chinese speaking. More than 700 active members are offered daily nutritious hot lunches as well as a variety of wellness, educational, art programs, and social services. The Older Adult center services provide an opportunity for older adults (age 60+) to lead more vibrant and meaningful lives through comprehensive programming and social support including social services, health promotion, and recreational/cultural/social opportunities case assistance and health promotion. The NORC serves residents age 60+ who are residents of the NYC Housing Authority developments Isaacs Houses and Holmes Towers by providing similar services as the OAC but with Case Management, Health Care Management, and Health Assistance services to support aging in place within the NORC.

Position Summary: The Aging Services Program Associate will

- Be responsible for providing coordination of services and outreach to older adults residing in and around the New York City Housing Authority development of Isaacs/Holmes Towers, Upper East Side, Yorkville, East Harlem and Beyond. - Develop, maintain and continually update a schedule of programs and supportive services in alignment with vision and contractual obligations of our DFTA Senior Center and SOFA/DFTA NORC. - Collaborate with agency staff to create intentional services with a focus on financial and housing stability, health and wellness, and food security and include access to arts and culture, education and recreation programs, and inter-generational activities and experiences. - Serve as liaison between specialists, consultants, volunteers and interns, supplemental service providers, property management, and other community stakeholders.- Assist with data entry and management to ensure correct reporting to funders.- Manage Volunteer Committee.- Work in collaboration with various departments including Aging Services, Food and Nutrition, and Education and Workforce.- Support day to day operations of the center.- Report directly to Program Coordinator

Schedule:Monday through Friday 35 hours per week.

Salary Range: $33,127- $38,649

Candidates must have the required qualifications to be considered for the position. The salary offer will be based on the candidate's number of years experience in the exact or comparable position and additional preferred qualifications will be taken into account.

Roles, Responsibilities, and Essential Duties

- Ensure participants gain access and opportunities to a whole range of social services, wellness education, socialization, arts and culture, and recreation activities through the programming at the Isaacs Center. - Assist Program Coordinator with the creation and management of a monthly calendar of activities, services, and events.- Develop and maintain relationships with community partners and consultants to bring new and innovative programming to the senior center in alignment with the goals and mission of the program.- Develop, maintain and continually update a directory of supportive services and agencies available to residents within the local community. Lead committees with participants including Inclusivity, Hospitality, Social Action, and Program Planning Committees. Coordinate regular trips with participants to cultural sights, as well as shopping and appointments- Support daily transportation coordination efforts from organizing, scheduling, record keeping, and occasional attendance on trips.- Assist with outreach efforts to increase participation. Ensure timely response and follow-up to referrals made. - Advocates for clients' rights, entitlements, and other benefits. Develop and foster strong relationships with consultants, community partners, and government agencies. - For participants who are hospitalized or in rehabilitation, communicate with the social worker to be informed of the care plan and participate in the discharge planning to assure appropriateness and availability of needed services are in place. - Provide front desk coverage when needed.- Assist with various programs as needed.- Participate in staff meetings, interdisciplinary case conferences, supervision, training, and more.- Work with supervisor in conjunction with the NYC Department for the Aging, New York State Office for the Aging, and Goddard Riverside to ensure that the appropriate processes for staff are in compliance with personnel policies and procedures

Qualifications & Educational Requirements:

- High School Diploma or equivalency required; Bachelor's degree preferred- Bilingual, English/Chinese or English/Spanish highly preferred- Prior experience working with the elderly highly preferred- Ability to handle confidential information with discretion- Ability to work some flexible hours (e.g., evenings, weekends, Thanksgiving or Christmas Holiday) - Working knowledge of community resources and service providers to older adults. - Ability to work effectively with culturally, economically and educationally diverse populations. - Ability to form positive interpersonal relations in dealing with a wide range of staff as well as external contacts- Strong verbal, written and interpersonal communication skills- Demonstrated knowledge of working with the elderly and knowledge of the process and issues of aging, including disability, mental health and abuse issues. Must be flexible in dealing with others. - Computer literacy including ability to use Microsoft Word, Excel, and Outlook
