Location: Emergency Room, Hospitals: South Seminole, Dr. Phillips, ORMC, South Lake, or St. Cloud
Schedule: 4 ten-hour shifts per week with rotating weekends and holidays
Position Overview:
We are seeking a dedicated and proactive Program Coordinator - Acquisition Coordinator to lead our patient acquisition efforts. This role is essential in identifying referral sources, selecting patients, reviewing referrals, and coordinating admissions to the Hospital Care at Home unit. The ideal candidate will have excellent communication skills, a keen attention to detail, and the ability to work effectively in fast-paced emergency room and inpatient unit environments.
Key Responsibilities:
- Identify Referral Sources: Establish and maintain relationships with referral sources to streamline the patient acquisition process.
- Patient Selection: Evaluate and select patients based on referral information and hospital criteria.
- Review Referrals: Assess and review patient referrals to determine eligibility and appropriateness for the Hospital Care at Home unit.
- Coordinate Admissions: Manage and coordinate the admission process for patients, ensuring a smooth transition to the Hospital Care at Home unit.
- Collaborate with Emergency Room Teams: Work closely with emergency room and inpatient nursing unit staff and other hospital departments to ensure effective patient flow and care coordination.
Qualifications:
Qualifications:
- Proven experience in healthcare coordination or a related field.
- Strong organizational and communication skills.
- Ability to work under pressure and manage multiple tasks simultaneously.
- Experience in patient admission processes and referral management preferred.
- Flexibility to work four ten-hour shifts per week, including rotating weekends and holidays.
- Current licensure as a registered nurse in the State of Florida or Nurse Licensure Compact (NLC) or as a licensed practical nurse in the State of Florida.
- One (1) year experience in the health care field.
- Associate degree, Nursing Degree from an accredited college or university, or graduate of an approved LPN program.
