This role plays an integral piece in our customers life by coordinating services for customers, to include, but are not limited to housing, finance, family relationships, social activities, education, employment, health, recreations, mobility, protective services, and records. Coordination of services also includes the development, implementation, and review of the Overall Service Plan (OSP). The OSP identifies the individualized support needed to achieve greater independence and participation in their community. The position has two main functions: provide support to customers in identifying and attaining life outcomes; and ensure supports provided by Community Options reflect the highest quality as defined by the accreditation standards from the Council on Quality and Leadership (CQL) and the ND DD Division. The salary for this position is $20 - $25 an hour depending on experience (DOE). QualificationsMinimum Qualifications:
- Bachelor's degree required, in Human Service, Developmental Disabilities, Social Work, Psychology, Behavioral Sciences, or related field preferred AND one (1) year experience in developmental disabilities (DD) or related field required. For any bachelor's degree not in the above related fields, incumbent must have DD certification completed upon hire.
- CPR/First Aid certified, or individual must successfully complete and become certified within initial 30 days of employment.
- A valid driver's license, acceptable driving record, reliable transportation, and proof of insurance for the vehicle subject to the statute of the licensing state will be required based on assigned job duties.
- Successfully pass a criminal background check, drug screening, and obtain ND Department of Human Service (DHS) pre-employment approval.
- Required to successfully complete required training and/or certification within specified time frame, includes DD certification; time requirements will vary based on assigned training or certification programs.
Essential Skills and Experience:
- Understanding of human services and developmental disabilities.
- Proficient computer skills including experience with Microsoft Office, Word, Excel, and PowerPoint, including basic accounting skills.
- Ability to communicate effectively in writing and orally.
- Demonstrated ability to lead people and get results through others; demonstrated team-building skills.
- Ability to plan over a one- to two-year time span.
