As a Program Coordinator, you'll oversee and coordinate operations within designated service delivery areas, including in-home and community-based services. You'll play a crucial role in developing and implementing Individual Support Plans, ensuring high-quality care, and maintaining compliance with policies and procedures. Key Responsibilities:
  • Develop and coordinate Individual Support Plans (ISPs) with measurable outcomes
  • Supervise and evaluate team members, ensuring proper training and implementation of care plans
  • Monitor service quality through regular visits, audits, and reviews
  • Manage budgets, track exceptions, and prepare variance reports for ISL programs
  • Coordinate medical care and promote wellness for individuals served
  • Facilitate community integration and natural support opportunities
  • Maintain accurate and timely documentation in compliance with HIPAA regulations
  • Conduct performance evaluations and provide leadership to direct support staff
  • Ensure all team members maintain current training and certifications
  • Collaborate with healthcare professionals to address medical needs and treatments
Education and/or Experience Qualifications:
  • Bachelor's degree from an accredited college or university
  • Relevant experience in the field of Developmental Disabilities can be substituted year for year for a degree
Additional Qualifications:
  • Must have appropriate, reliable transportation with working air conditioning and heating
  • Valid driver's license, proof of insurance, and clean driving record required
  • Strong communication and interpersonal skills
  • Proficiency in computer systems and documentation software
  • Ability to work on-call according to the regional office schedule
  • Physical ability to perform CPR, First Aid, and assist with client transfers as needed
  • Demonstrated leadership and problem-solving skills
